Teeth grinding and other biting habits are more than a nuisance — they can generate twenty to thirty times the forces of normal biting. Over the long term, this can cause significant damage to teeth and supporting gums and bone.
This particular kind of damage is known as occlusal trauma (meaning injury from the bite). In its primary form, the habit itself over time can injure and inflame the jaw joints leading to soreness, swelling and dysfunction. The teeth themselves can wear down at a much faster rate than what normally occurs with aging. And although less common but even more serious, the periodontal ligaments holding teeth in place to the bone can stretch and weaken, causing the teeth to become loose and increasing the potential for tooth loss.
There are a number of techniques and approaches for treating excessive biting habits, but they all have a common aim — to reduce the amount of force generated by the habit and the associated problems that result. A custom occlusal guard, often worn while sleeping, helps lessen the force by keeping the teeth from making solid contact with each other. Tissue soreness and swelling can be relieved with anti-inflammatory drugs like aspirin or ibuprofen, muscle relaxants or physical therapy. In cases where stress is a main driver, behavioral therapy and counseling may also be helpful.
Biting forces are also an issue for patients with periodontal (gum) disease. In this case even biting forces within normal ranges can cause damage because the diseased gums and bone have already been weakened. If gum disease is a factor, the first priority is to treat the disease by removing built up plaque. Plaque is the thin film of bacteria and food remnant that’s both the cause and continuing growth of the infection, as well as tartar (calculus) from all tooth and gum surfaces.
A thorough dental exam will reveal whether a tooth grinding habit is playing a role in your teeth and gum problems or if it’s magnifying the damage of gum disease. In either case, there are appropriate steps to stop the damage before it leads to tooth loss.

If there's anything that makes Alfonso Ribeiro happier than his long-running gig as host of America's Funniest Home Videos, it's the time he gets to spend with his family: his wife Angela, their two young sons, and Alfonso's teenaged daughter. As the proud dad told Dear Doctor–Dentistry & Oral Health magazine, "The best part of being a father is the smiles and the warmth you get from your children."
Because Alfonso and Angela want to make sure those little smiles stay healthy, they are careful to keep on top of their kids' oral health at home—and with regular checkups at the dental office. If you, too, want to help your children get on the road to good oral health, here are five tips:
- Start off Right—Even before teeth emerge, gently wipe baby's gums with a clean, moist washcloth. When the first teeth appear, brush them with a tiny dab of fluoride on a soft-bristled toothbrush. Schedule an age-one dental visit for a complete evaluation, and to help your child get accustomed to the dental office.
- Teach Them Well—When they're first learning how to take care of their teeth, most kids need a lot of help. Be patient as you demonstrate the proper way to brush and floss…over and over again. When they're ready, let them try it themselves—but keep an eye on their progress, and offer help when it's needed.
- Watch What They Eat & Drink—Consuming foods high in sugar or starch may give kids momentary satisfaction…but these substances also feed the harmful bacteria that cause tooth decay. The same goes for sodas, juices and acidic drinks—the major sources of sugar in many children's diets. If you allow sugary snacks, limit them to around mealtimes—that gives the mouth a chance to recover its natural balance.
- Keep Up the Good Work—That means brushing twice a day and flossing at least once a day, every single day. If motivation is an issue, encourage your kids by letting them pick out a special brush, toothpaste or floss. You can also give stickers, or use a chart to show progress and provide a reward after a certain period of time. And don't forget to give them a good example to follow!
- Get Regular Dental Checkups—This applies to both kids and adults, but it's especially important during the years when they are rapidly growing! Timely treatment with sealants, topical fluoride applications or fillings can often help keep a small problem from turning into a major headache.
Bringing your kids to the dental office early—and regularly—is the best way to set them up for a lifetime of good checkups…even if they're a little nervous at first. Speaking of his youngest child, Alfonso Ribeiro said "I think the first time he was really frightened, but then the dentist made him feel better—and so since then, going back, it's actually a nice experience." When your favorite baseball team wins, it's hard not to get excited — especially if you're right there in the stadium. It's even better when a player tosses the ball to fans. But sometimes, in the heat of the moment, things can go awry.
That's what happened during a recent game at New York's Yankee Stadium. After catching the ball that ended the game in an 8-2 Dodgers win, Los Angeles outfielder Yasiel Puig tossed it into a cheering crowd of supporters. “I saw it coming at me and I remember thinking, 'I don't have a glove to catch this ball,'” Dodgers fan Alyssa Gerharter told the New York Daily News. “I felt it hit me and I could feel immediately with my tongue there's a hole. And I looked down at my hand and saw there's a tooth in my hand.”
Ouch. Just like that, one fan's dream became… a not-so-good dream. But fortunately for the 25-year-old software engineer, things went uphill from there. Ushers quickly escorted her into a first-aid room at the stadium. She was then rushed to a nearby hospital, where the upper front tooth was re-inserted into her jaw. After a follow-up appointment at her dentist's office the next day, Gerharter said she remains hopeful the re-inserted tooth will fuse with the bone, and won't require replacement.
We hope so too. And in fact, she has as good a chance of a successful outcome as anyone, because she did everything right. If you're not sure what to do about a knocked-out tooth, here are the basics:
- locate the tooth, handle it carefully (don't touch the root surface), and if possible gently clean it with water
- try to open the person's mouth and find the place where the tooth came from
- carefully re-insert the tooth in its socket if possible, making sure it is facing the right way
- hold the tooth in place with a soft cloth as you rush to the dental office or the nearest urgent care facility
- if it can't be replaced in its socket, place the tooth in a special preservative solution or milk, or have the person hold it between the cheek and gum (making sure they won't swallow it) — and then seek immediate care at the dental office
- follow up at the dental office as recommended
In general, the quicker you perform these steps, the more likely it is that the tooth can be preserved. How quick is quick? The best outcomes are expected when re-implantation occurs in no more than five minutes. So if you're in this situation, don't wait: get (or give) appropriate first aid right away — it just might save a tooth!

November is National Diabetes Month—a time to focus on a disease that affects more than 400 million people around the world. What does diabetes have to do with oral health? Plenty! Here's a true-or-false quiz to test your knowledge on this important topic.
TRUE OR FALSE:
1. Diabetes and gum disease are connected.
TRUE. Studies have found a clear association between diabetes and gum (periodontal) disease, especially when diabetes is not well controlled. People with poorly controlled diabetes have a more severe inflammatory response to the bacteria that cause gum disease. While inflammation is normally a protective reaction of the body's immune system, too much inflammation can actually make the condition worse. In the case of gum disease, the reverse is also true: Untreated gum disease can worsen blood sugar levels in people with diabetes. The good news is that treatment of periodontal disease has been shown to improve blood sugar control.
2. People with diabetes can't have dental implants.
FALSE. Research has shown that dental implants can be a very successful tooth-replacement treatment for people with diabetes. But again, blood sugar control can be a factor. Dental implants are titanium posts that serve as artificial tooth roots. Minor surgery is required to insert an implant into the bone beneath the gums; a realistic-looking dental crown is later attached to it so it can look and function like a natural tooth. Studies have shown that it takes longer for the bone to heal around implants in people with poorly controlled diabetes. That doesn't make implant treatment impossible, but it does mean that it may be managed differently. For example, an implant may be allowed to heal for a longer period of time before a crown is attached to it.
3. People with diabetes can't do anything to improve their oral health.
FALSE. People with diabetes can have a very positive impact on their oral heath, by doing their best to control blood sugar levels with a healthy diet and exercise, and by sticking to an effective daily oral hygiene routine. This includes brushing twice a day for two minutes each time, and flossing at least once each day to remove bacterial plaque between teeth. Regular dental checkups and cleanings are also essential—not just for people with diabetes, but for everyone!

The chances of contracting an infectious disease from a dental visit are extremely low, thanks to the stringent safety standards practiced by over 170,000 dental care providers across the U.S. Without these standards, you and your family would be at risk for diseases like hepatitis from even a routine office visit.
The main prevention focus centers on blood-borne diseases in which blood from an infected person is introduced into the body of another through a cut, incision or injection site. While HIV/AIDS (autoimmune deficiency syndrome) is perhaps the most well known of blood-borne diseases, a more common and thus a more threatening disease is hepatitis. Caused by a pair of viruses known as HBV and HCV, hepatitis damages the liver, which disrupts normal bodily function and can even cause death.
The spread of hepatitis and similar diseases is a major concern for blood transfusion and surgical centers that commonly use invasive procedures and intravenous (IV) equipment. It’s also a concern in dental offices where even a hygienic cleaning may result in some bleeding. To reduce the risk of disease, the dental profession has several layers of both mandatory and recommended standards for protection against viral or microbial transmission.
The Center for Disease Control, for example, publishes and regularly updates recommended procedures for equipment sterilization and disinfection. State level dental licensing boards also mandate safety procedures and require continuing education for infection control as a requirement for re-licensing, as often as two years. Professional organizations such as the American Dental Association (ADA) also encourage safety protocols among its members.
The vast majority of dentists place infection control among their highest priorities. These care providers institute and practice daily protocols and procedures for hand washing, use of masks, gloves and other biohazard protection, and disinfection. Through effective infection control you and your family can receive the dental care you need without endangering your general health.
